JOB DESCRIPTION AND DETAILS
Summary of Position Under minimal supervision, the Medical Records Coder will perform remote coding services on Outpatient clinic encounters and identify to Coding Management team any inappropriate patterns of documentation.
This is a Service Contract Act (SCA) covered position.
Essential Duties and Responsibilities
Review and verify documentation to ensure completeness and accuracy of encounter.
Select appropriate diagnoses, evaluation and management, procedural codes, along with modifiers and units of service based on documentation reviewed.
Provide discrepancies, administrative errors, or inappropriate patterns of documentation to Coding Management team.
Receive audits from Coding Management team, review and provide justification for errors found based on the ICD-9/ICD-10 coding books, ICD-10 Coding handbook, Coding Clinics, CPT (Current Procedural Terminology) Assistant and/or CCE equivalent references, and Military Health Systems (MHS) Professional Coding Guidelines
Outpatient Encounters: 12 per hour
Emergency Room encounters: 8 per hour * Accuracy Standard: 95% in Diagnoses, Evaluation and Management, and Procedural categories
Limited overtime possibilities based on workload
No travel required for this position.
Certified Medical Coder(s), Certified Coding Specialists (CCS), Certified Coding Specialist(s)-Physician based (CCS-P), Certified Professional Coder(s) (CPC), Registered Health Information Administrator(s), (RHIA) and Registered Health Information Technician(s) (RHIT) as necessary to code medical record data
Must be able to attain and maintain a favorable adjudication for a National Agency Check with Inquiries (NACI) background check
Proficiency in Microsoft applications such as: Outlook, Word, Excel, and PowerPoint
Ability to meet and/or exceed standards in accuracy and productivity
Time Management Skills
Strong Communication and Interpersonal Skills
Preferred experience with AHLTA/CHCS and 3M CCE.
Ability to analyze, problem-solve, and work independently effectively.
CCE/ AHLTA/ Essentris/ CHCS/ 3M knowledge preferred.
Knowledge of current and developing issues and trends in medical coding.
Attention to detail and data entry accuracy is essential.
Must be able to sit, verbally converse and work on a computer for long periods of time with or without reasonable accommodation
Frequent use of hands/fingers across keyboard or mouse or handling other objects.
